John "Jay" Collins

John “Jay” Collins, Jr., of Bowling Green, KY, passed away on December 12, 2017 at Pilgrim Manor in Plymouth, Indiana. Jay was born on May 25, 1952 in Mishawaka, IN to the late John A. Collins Sr. and Madge M. (Hayden) Collins. He married Gail (Wilborn) Collins on December 16, 1972.  She preceded him in death on June 15, 2014. Formerly of Lakeville, IN, he was the President of South Bend Orthodontics and OrthoXpand International. He served honorably in the United States Navy and attended Faith Baptist Church in Plymouth, Indiana.

Jay is survived by his daughter, Jamie (Tony) Palmer of Plymouth, Indiana; his son Lance (Laura) Collins of Ashville, Ohio; and eight grandchildren, Gabrielle Collins, Lliam Collins, Isabelle Collins, Madellyn Collins, Sydney Palmer, A.J. Palmer, Preston Cleveland, and Tyler Palmer.

Memorial services for Jay will take place at a later date. The family is in the care of Palmer Funeral Home – Lakeville.  Donations in honor of John “Jay” Collins, Jr. may be given to Faith Baptist Church, 12987 Peach Road, Plymouth, IN 46563.
