Azariah Marie Hernandez

Azariah Marie Hernandez, 7 months old, passed away on Thursday, August 9, 2018 at her home.  She was born on January 15, 2018 in Elkhart, IN to Charles Lewis and Oriana Hernandez. 

Azariah is survived by her parents; three sisters, Nevaeh Hernandez, Kaiden Wagner and Mariyah Houpt; brother, Darion Houpt; grandparents, Felipe Hernandez, Dawn Buck, Charles McClinton and Tina Lewis; and great grandparents, Felipe (Manuela) Hernandez.

Funeral services will be held at 12:00 p.m. Monday, August 20, 2018 at Palmer Funeral Home-Guisinger Chapel, 3718 S. Michigan St. South Bend, IN.  Graveside services and burial will follow at Rice Cemetery, Elkhart, IN.  Friends may visit with the family from 10:00 a.m. - 12:00 p.m. Monday in the funeral home.
